Pioglitazone: From discovery to clinical practice Essay

The most common type of diabetes is Type II diabetes mellitus which occurs in more that 90% of the cases of diabetes reported (Thomas, 2013). Type I diabetes is not as common  and is caused by destruction of the islet of Langerhans due to susceptibility of an autoimmune destruction leading to a reduced amount of insulin produced and glucose intolerance (Feinglos,2008). Type I diabetes is mostly diagnosed in children and young adults. Unlike diabetes type  I, Type II diabetes mellitus is mostly diagnosed in older people and is caused by unhealthy lifestyles (Thomas, 2013). Type II diabetes is characterized by production of insulin which does not perform its function. It in turn leads to a situation referred to as insulin resistance. The patients will suffer from low levels of insulin which functions as   a component  for the regulation of body sugar (Feinglos, 2008). The number of islets of Langerhans in the pancreas is normal or somewhat reduced. It is mostly characterized by hyperglycemia that is associated with the insufficient or dysfunctional insulin present in the body (Thomas, 2013). Abnormal coagulation, hypertension and diabetic dyslipidemia are also commonly reported to signify the occurrence of type II diabetes mellitus. The increased number of obesity cases has seen the diagnosis of diabetes mellitus type II in teenagers and young adults (Feinglos, 2008). In cases when the patient is not obese, there is a presence of high fat distribution in the body (Feinglos, 2008). People suffering from Type II diabetes mellitus, unlike type I diabetes, do not need regular injections of insulin (Thomas, 2013). The management of type II diabetes involves the control of the increased levels of blood sugar. It is, however, important to note that the reduction of the sugar levels should not surpass the normal blood sugar levels. Because most of the patients are overweight, exercise, a proper balanced diet and reduction of weight is fundamental in the management of type II diabetes mellitus (Feinglos, 2008). Pioglitazone                  Pioglitazone is a drug belonging to a class of thiazolidinediones and is used in the management and of type II diabetes mellitus (Schatz, 2000). Since the body is unable to control the increasing levels of sugar in the blood, pioglitazone is used to increase the sensitivity to insulin (Feinglos, 2008). The drug, however, does not cure the diabetes mellitus but only helps the body in maintaining  normal  blood sugar levels. It is an oral formulation and can be taken twice daily with or without taking meals (Schatz, 2000). It is able to achieve this by inhibiting the hepatic gluconeogenesis process and also increase peripheral and splanchnic glucose uptake which implies that there is a minimal occurrence of increased blood sugar levels (Feinglos, 2008). It effects can be seen in the short term and also in the long-term (Schatz, 2000). The use of pioglitazone can be done according to the doctor’s prescription. It also important to inform the docto r before stopping the use or if any complications arise while using pioglitazone.The development of pioglitazone is based on the discovery of gene SOCS3 (Suppressor of Cytokine Signaling-3) (Schatz, 2000). The elevation of the gene causes interferon resistance that in turn causes insulin resistance in the liver. Depletion of this gene has been made possible by pioglitazone which results in the reduced insulin resistance in the liver although it may cause systemic insulin resistance (Schatz, 2000). The suppression of gene SOCS3 by pioglitazone gives it the antiglycemic and ant diabetic property hence it has bee used in the management of the two cases. References Thomas, M. Linear Model of Communication Essay

It is a one way model to communicate with others. It consists of the sender encoding a message and channeling it to the receiver in the presence of noise. Draw backs – the linear model assumes that there is a clear cut beginning and end to communication. It also displays no feedback from the receiver. †¢ For example; a letter, email, text message, lecture. There are two main linear model which are used to explain the human communication process. They are †¦ †¦ †¦ †¢ Shannon and Weaver Model (1949) The original model was designed to mirror the functioning of radio and telephone technologies. Their initial model consisted of three primary parts: sender, channel, and receiver. The sender was the part of a telephone a person spoke into, the channel was the telephone itself, and the receiver was the part of the phone where one could hear the other person. Shannon and Weaver also recognized that often there is static that interferes with one listening to a telephone conversation, which they deemed noise. The noise could also mean the absence of signal. †¢ Berlo’s SMCR Model (1960) In 1960, David Berlo expanded on Shannon and Weaver’s linear model of communication and created the SMCR Model of Communication. The Sender-Message-Channel-Receiver Model of communication separated the model into clear parts and has been expanded upon by other scholars. Shannon – Weaver Model Claude Elwood Shannon (April 30, 1916- February 24,2001) was an American mathematician, electronic engineer and cryptographer.He is known as â€Å"the father of information theory†. He was a student of Michigan University. He was a graduate in both engineering and mathematics. He worked in Bell laboratories, Massachusetts Institute of Technology, Institute for Advanced Studies.He got IEEE Medal of Honor, Kyoto Prize, Harvey Prize awards. [pic] Warren Weaver (July 17, 1894- Nov 24, 1978) was an American Scientist, Mathematician and Science administrator.He is widely recognized as one of the pioneers of machine translator and as important figure in creating support for science in US. He was a student of University of Wisconsin- Madison. Introduction: Shannon- Weaver model is mother of all model in communication. The theory based on how a concept goes to a place from another. It is also known as mathematical theory of communication. Shannon published â€Å"A Mathematical Theory of Communication† article in two parts in the July and October numbers of the â€Å"Bell System Technical Journal† in 1949. The book co-authored with Weaver â€Å"The Mathematical Theory of Communication† reprints Shannon’s article. Later Weaver explained the model and popularized it. Formula: C = W log2(1+S/N) where, C – Channel capacity W – Bandwith S – Signal level N – Noise power This model is specially designed to develop the effective communication between sender and receiver. Also they find factors which affecting the communication process called â€Å"Noise†. At first the model was developed to improve the Technical communication. Later it’s widely applied in the field of Communication. Sender : Mr. Khan Encoder : Telephone (Mr. Khan) Channel : Cable Noise : Distraction in voice Reception : Telephone (Assistant) Receiver : Assistant. Due to transmission error or noise, Assistant can’t able to understand Thomson’s messages. Criticism: There are also criticism of Shannon- Weaver model. †¢ One of the simplest model and its general applied in various communication theories. †¢ The model which attracts both academics of Human communication and Information theorist to leads their further research in communication. †¢ It’s more effective in person-to-person communication than group or mass audience. †¢ The model based on â€Å"Sender and Receiver†. Here sender plays the primary role and receiver plays the secondary role (receive the information or passive). †¢ Communication is not a one way process. If it’s behaved like that, it will lose its strength. For example: Audience or receiver who listening a radio, reading the books or watching television is a one way communication because absence of feedback. †¢ Understanding Noise will helps to solve the various problems in communication. Conclusion: Basically Shannon made this model not for the human communication. Rather he conceived the mediated communication, to be explicit the very phony conversation. Later human communication scholars began to use it to explain the unmediated human communication, as it is very simple and not far away. Berlo’s SMCR Model [pic] Dr. David K. Berlo (1929- 1996) was an American communication experts. He is called as â€Å"the father of modern mass communication†. Dr. Berlo explained his model as an expansion of Shannon- Weaver model in 1960 in his book â€Å"The Process of Communication†. He was a professor of department of communication, Michigan State University (MSU). Introduction: The model of David Berlo is known as â€Å"Berlo’s SMCR model†. He is the one who transfers the information to the receiver after carefully putting his thoughts into words. The receiver should be on the same platform as the speaker for smooth flow of information and better understanding of the message. He should possess good communication skills to understand what the speaker is trying to convey. He should have the right attitude to understand the message in a positive way. His knowledge should also be at par with the listener and must know about the subject. He should also be from the same social and cultural background just like the speaker. Interpretation and Significance of Lao Tzu's Sayings - Essay Example Taoists have developed their principles through the processes of self- realization and self-invention based on a variety of philosophical and environmental contexts. Like other organized religion, Daoism has its own doctrines or scriptures for its followers. In their collaborate effort Yijie Tang, and TÊ »ang I-chieh have rightly put it: â€Å"The early form of Daoism (Taoism) held that its body of belief was made up of the tenet of â€Å"the ascent of the three in one â€Å"that is,† the unity of heaven, earth, and man for the attainment of the Great Peace; the blending of the essence, breath and shen to become a saint.† (Yijie Tang, and TÊ »ang I-chieh, 1991, 76 pp.). Daoism (Taoism) is familiar to the world through the famous books Chuang-Tzu and Tao Te Ching. Daoist sayings are more comprehensive, filled with metaphysical principles and these principles emphasis a formation of ‘oneness’ that follow the complex form of diverse world. The book had wide ly accepted as the great sayings of Lao Tzu. It has considered as the unique ideology in Chinese history. Lao Tzu explores his ideas through the help of rhyme and rhythm, paradox, analogy, and proportion. In his masterpiece Lao-Tzu remarks, every people have his/her own freewill and desires. The Chinese word Tao/ Dao means the way to the universe. When analyzing these sayings, one can understand that it is difficult to define these sayings appropriately. In the first part of this work, the author tries to give the foundation about the ideology Tao Te Ching. When analyzing the plight of modern man one can easily find that the sayings of Lao-Tzu have enough relevance. The temporariness of material things and pleasures have well expressed in the sayings of Lao Tzu. The followers of Lao-Tzu purport that the ultimate aim of life is to gain the ‘Great Peace’. One cannot attribute it a clear form or structure that it may appear in any name or material existence.

This reduces their transaction cost but also increases the credit risk. When an intermediary is introduced in the same model as in case of exchange traded funds, the transaction cost increases due to brokerage charges, maintenance fee, printing and advertising charges, commissions, and so on. The various costs are passed on to the fund raiser and hence decreasing the transaction cost of the individual from intermediation process (Buckle and Thompson, 2004, p.37).
